Fountain Park

Fountain Park is an inviting Assisted Living, Memory Care, and Villas community located in the charming small town of Bryan, Ohio.

Our beautifully landscaped grounds offer an ideal setting for relaxing outdoors. We’re just minutes from Michigan and Indiana, in a booming downtown shopping district, making Fountain Park the perfect place to call home.

We provide a cozy, home-like atmosphere, with all the advantages of living in an active community, enhancing our seniors’ lives. From concerts, holiday celebrations, parties, and trips to local restaurants and Wii-bowling tournaments – our residents are constantly stimulated with activities of their choosing. We encourage participation in as many activities as possible, in a relaxed environment.

We offer a wide range of services to give our seniors the right level of care for their situation: a flexible adult daycare and respite stays for up to six weeks that can serve as a trial period to see if Fountain Park is a good fit for your loved one.

We Offer Memory Care
Fountain Park also offers a Memory Care wing with specially trained staff for dementia and Alzheimer’s residents with Wanderguard protection, to ensure that your loved ones are safe and secure.

Purpose
The purpose of our organization is to promote the spiritual, physical, social, psychological, and emotional wellness of each resident.

Certification
Fountain Park Assisted Living is certified by ODA to provide the services under the assisted living program. Fountain Park Assisted Living is currently accepting individuals who are enrolling in the assisted living program.

“Our family was very pleased with the care that Ramona received at Fountain Park. The staff was always so considerate of all her needs. We never needed to worry about the care she was receiving. The nurses made sure that she received her meds and eye drops at prescribed times. She needed assistance to get to her “reading machine” and to the bathroom and all she needed to do was to pull her cord and they were soon there to assist her. They were willing to get her a snack out of her refrigerator every evening and tuck her into bed and adjust her pillows to her liking. As a matter of fact, she told me once that an aide asked her what she thought heaven would be like and Ramona responded “well like Fountain Park” because everyone took care of her every need. Ramona was 100 years old when she entered Fountain Park for a brief stay In the respite room. While there, we talked daily about whether she would go home or stay at Fountain Park. She decided that being there was much better than depending on home health care. She chose that particular assisted living facility because, even when she was more mobile, they would even assist her to the dining room. I only remember her complaining about the food ONCE and she was there until she passed away at age 105.” – JoAnn Luce

The nursing and aid staff are very attentive to the residents and alert family immediately in any change in health, personality, or actions. This allows the family to evaluate immediately if the resident needs a DR. appt, trip to ER or just to see if something is upsetting them so the family can address any issues the resident may have.

The Church services, Entertainment, and activities available are great. This allows and encourages the residents to interact and feel like a community.
